Referral Bonus Disclaimer

Davin Healthcare Employee Disclosure: *The referral bonus of $1,000.00 is subject to income tax and will be processed in the week following the referred caregiver’s successful completion of their 13-week contract. The referral bonus of $100.00 is subject to income tax and will be processed in the week following the referred caregiver’s after working 6 days on a per diem contract. Referral bonuses paid to current Davin Healthcare employees will be paid out through payroll, subject to all applicable withholding taxes. 

Non-Employee Disclosure: **The referral bonus of $500.00 is subject to federal and state income tax and will be processed in the week following the referred caregiver’s successful completion of their 13-week contract. The referral bonus of $100.00 is subject to income tax  and will be processed in the week following the referred caregiver’s after working 6 days on a per diem contract. The referring party will be responsible for reporting the amount received on their tax returns.
